Under the direction of the Department Manager the Community Healthcare Worker provides culturally appropriate general health education including but not limited to health, disease, and disabilities in American Indian Alaska Native (AIAN) communities. The position is responsible for client focused outreach and home visit activities which supports medical and/or behavioral health services, as assigned. The CHW coordinates with integrated healthcare staff (e.g., medical, family advocate, behavioral health staff, etc.) to support clients by linking them to Developmental Services and or medical, behavioral health, and community-based resources/services. The CHW provides case management and data tracking of Developmental Outreach and Engagement and screening and assessment.

The CHW position encompasses a wide scope of duties including data entry to electronic health records and spreadsheets. The CHW primary role is to identify, case manage, and coordinate services for Native Americans who may be eligible to receive services under IEP assessment criteria for developmental services. This includes assisting clients with applying for health insurance, scheduling appointments, and assisting with transportation of child, youth, and their family, as determined by IEP and multi-disciplinary treatment team.

· Answer phones and respond to patient messages in a professional manner.

· Collect pertinent information regarding insurance coverage, tribal or Native American affiliation.

· Clearly and concisely communicate with the patient and parent/guardian regarding pre and post care steps to services.

· Properly document all patient charts timely and upload all pertinent records expediently.

· Facilitate the development of wellness plan focused on strategic goals skills related to IEP.

· Serve as an advocate providing clients information and support and linkages to all services available.

· Assist with children and youth services and care coordination / case management.

· Recommend and make referrals to programs for children and youth and collaborate with all servicing agencies providing care to individuals with developmental disabilities.

· Case manages clients in school-based programs, in group settings, or individual therapy, and outreaches and engages children, youth, and their family in their place of residence, as appropriate.

· Monitor and keep case records on clients and report progress to supervisors; participate in wellness checks and assessments.

· Attend required staff meetings and training as assigned.

· Maintain client charts in a responsible manner and meet all report deadlines as required by funding and accrediting agencies.

· Provides client transportation and participates in events to engage the population of focus.

· Communicates effectively with all members of the multi-disciplinary health team.

· Alerts nursing staff immediately to any signs of patient distress or health care concerns.

· Maintains regular charts of client medical issued equipment use and reports equipment needing repairs or upgrades.

· Complies with regulatory agency standards, including federal, state, and Accreditation Association of Ambulatory Health Care framework.

· Adheres to facility confidentiality and patient’s rights policy as outlined in the facilities Health Insurance Portability and Accountability Act (HIPAA) policies and procedures.

· Promotes vaccine awareness and refers as needed.

· Performs community outreach via face-to-face, phone calling, and or uses virtual platforms for education and instruction, as needed.

· Performs other duties as assigned.
